What to do if you need hospital care …

Emergency Department Care:

If you are in any facility for emergency cardiac care, it is very important that you insist on notification of Upstate Cardiology for our cardiologist on call.  The medical personnel in the Emergency Department may want to contact a different cardiology group, but we ask that you insist on calling Upstate Cardiology. Our physicians will have immediate access to the complete medical information in your electronic medical record.  Ready access to these records can help ensure that you get the most appropriate care in the quickest time.  Please call our office immediately if hospital personnel do not honor your request for the Upstate Cardiologist on call.

Upstate Cardiology Hospital Treatment Team

The Upstate Cardiology Hospital Treatment Team is made up of our cardiologists, nurse practitioners and physician assistants.  Our goal is to work together in diagnosing and treating your cardiovascular disease.

What is a Nurse Practitioner or a Physician Assistant?

Nurse Practitioners and Physician Assistants are health care professionals licensed to practice medicine with physician supervision.  Our NPs and PAs are used in the hospital setting as well as the office setting. They may conduct physical exams, diagnose and treat illnesses and order and interpret tests.  NPs and PAs can also write prescriptions, assist with hospital admissions and discharges, and attend to your medical education needs.

HOSPITAL STAYS

WILL THE SAME PROVIDER SEE ME EACH DAY?

You will be seen every day by one of our cardiologists.  You may also see one of our nurse practitioners or physician assistants.  Due to the cardiologist’s varying schedules, you may not see the same cardiologist each day.

Our providers meet every morning to discuss your diagnosis and treatment progress. Before a provider visits with you, he or she has reviewed your medical records. Your personal treatment goals will be  discussed with you daily during your hospital stay.

WHEN WILL I BE NOTIFIED OF MY FOLLOW-UP OFFICE
APPOINTMENT?

At discharge, you will know the date and time of your follow-up appointment. It will be written or typed on your discharge paperwork. If you are a current patient of Upstate Cardiology, your appointment will be with your regular cardiologist. If you are a new patient, your appointment will be scheduled with the cardiologist who admitted you.

WHAT SHOULD I DO IF I HAVE A QUESTION OR CONCERN PRIOR TO MY SCHEDULED FOLLOW-UP APPOINTMENT?

Please call our office at 235-7665. We have staff dedicated to answer your questions and to facilitate a visit prior to your scheduled appointment if necessary.  If you need to be seen urgently, we can accommodate same day patients.  In this situation, you may be seen by a cardiologist other than your regular one.  Our goal is to get you the care you need at the time you need it.
